Narrative:
Crew arrived from rest facility and conducted preflight. Discovered need for de-ice. Advised of ATC delay into den. Realizing de-ice would be slow because my company uses other air carrier's equipment, I specified to ATC a depart time later than the one assigned. Was advised by ATC that if the final time was missed, we would have to reapply for depart time. Company late boarding and de-icing (lack of personnel). Started engines with 2 mins to go, rushing procedures. Taxied to runway 35 (toward den) when had been cleared to runway 17. Turned wrong way, leaving ramp. Tower had pre-issued takeoff clearance, but cancelled it when taxi error discovered. More confusion with tower as to intersection takeoff acceptable or not. Distracted by F/a with passenger count. Took off, and to this moment, I do not remember being cleared for takeoff. This had the potential for a 'canary islands' takeoff accident. Company procedure for F/a cockpit visit while taxiing contributed, but the blame is entirely mine. Why try so hare to get the job done?? I hope this teaches me to never hurry again--even if the delay was my fault.

Narrative: CREW ARRIVED FROM REST FAC AND CONDUCTED PREFLT. DISCOVERED NEED FOR DE-ICE. ADVISED OF ATC DELAY INTO DEN.
