Narrative:
I received clearance on frequency 118.0 to takeoff on runway 30R VFR to 4000' with transponder setting. After receiving this information I was told to go to ground frequency 121.8. I did and told them I had clearance to takeoff on 30R with the above information. I was told to taxi to runway 30R and ready for takeoff. I taxied to taxi holding area and looked to see if any aircraft was in pattern and thinking I was on tower frequency proceeded to takeoff on 30R. After taking off I checked my radios and found I was on the wrong radio and hot tower frequency. I changed to the tower frequency and called in. They instructed me to land on 30R and call upon parking aircraft. I described the issue and told them I was not intending to break the rules, but thought I was doing everything right, until I found out about the radio being tuned to the wrong frequency. The problem of switching from 118.0 to 121.8 to 120.7 with a select system has its problems of knowing what radio you are talking on.

Title: GA SMA MADE AN UNAUTH TKOF FROM RWY 30R AT SJC. PLT WAS STILL ON ATCT GND CTL FREQ AND FAILED TO CALL ATC LCL CTL.
Narrative: I RECEIVED CLRNC ON FREQ 118.0 TO TKOF ON RWY 30R VFR TO 4000' WITH XPONDER SETTING. AFTER RECEIVING THIS INFO I WAS TOLD TO GO TO GND FREQ 121.8. I DID AND TOLD THEM I HAD CLRNC TO TKOF ON 30R WITH THE ABOVE INFO. I WAS TOLD TO TAXI TO RWY 30R AND READY FOR TKOF. I TAXIED TO TAXI HOLDING AREA AND LOOKED TO SEE IF ANY ACFT WAS IN PATTERN AND THINKING I WAS ON TWR FREQ PROCEEDED TO TKOF ON 30R. AFTER TAKING OFF I CHKED MY RADIOS AND FOUND I WAS ON THE WRONG RADIO AND HOT TWR FREQ. I CHANGED TO THE TWR FREQ AND CALLED IN. THEY INSTRUCTED ME TO LAND ON 30R AND CALL UPON PARKING ACFT. I DESCRIBED THE ISSUE AND TOLD THEM I WAS NOT INTENDING TO BREAK THE RULES, BUT THOUGHT I WAS DOING EVERYTHING RIGHT, UNTIL I FOUND OUT ABOUT THE RADIO BEING TUNED TO THE WRONG FREQ. THE PROB OF SWITCHING FROM 118.0 TO 121.8 TO 120.7 WITH A SELECT SYS HAS ITS PROBS OF KNOWING WHAT RADIO YOU ARE TALKING ON.
