Narrative:
During climb out from lgw, crew noticed illumination of #3 engine pneumatic high pressure light, concurrent with illumination of #3 reverser valve open and #3 pack off lights. Secured #3 engine pneumatics. Shortly thereafter, a F/a reported that a passenger had observed what appeared to be a missing access panel from the #3 engine pylon area. Second officer inspected #3 engine area and confirmed missing panel. Contacted lgw operations/maintenance. Informed them of indications and suggested a runway sweep by performed for possible contamination. None found. Contacted company maintenance on HF. Informed them of all indications and action taken. All aircraft indications and flight conditions remained normal. Relayed same to company maintenance approximately each 1 1/2 hours during flight. Proceeded to den (destination) and, after gate arrival, unable to shut down #3 engine. Called maintenance to secure engine. Preliminary inspection revealed faulty duct/clamp on #3 engine pneumatic system. Still awaiting final disposition of maintenance inspection and correction.

Narrative: DURING CLBOUT FROM LGW, CREW NOTICED ILLUMINATION OF #3 ENG PNEUMATIC HIGH PRESSURE LIGHT, CONCURRENT WITH ILLUMINATION OF #3 REVERSER VALVE OPEN AND #3 PACK OFF LIGHTS. SECURED #3 ENG PNEUMATICS. SHORTLY THEREAFTER, A F/A RPTED THAT A PAX HAD OBSERVED WHAT APPEARED TO BE A MISSING ACCESS PANEL FROM THE #3 ENG PYLON AREA. S/O INSPECTED #3 ENG AREA AND CONFIRMED MISSING PANEL. CONTACTED LGW OPS/MAINT. INFORMED THEM OF INDICATIONS AND SUGGESTED A RWY SWEEP BY PERFORMED FOR POSSIBLE CONTAMINATION. NONE FOUND. CONTACTED COMPANY MAINT ON HF. INFORMED THEM OF ALL INDICATIONS AND ACTION TAKEN. ALL ACFT INDICATIONS AND FLT CONDITIONS REMAINED NORMAL. RELAYED SAME TO COMPANY MAINT APPROX EACH 1 1/2 HRS DURING FLT. PROCEEDED TO DEN (DEST) AND, AFTER GATE ARR, UNABLE TO SHUT DOWN #3 ENG. CALLED MAINT TO SECURE ENG.
