Narrative:
Small aircraft X on approach to osu airport, pointed out to cmh tower, this is to maintain separation. Tower turned small transport Y 300 degree, aircraft climbed to 17' leveled. Separation deteriorated to 2 NM at 17' 3 east of osu before small transport Y was found still on tower frequency. To resolve, tower must follow SOP.

Narrative: SMA X ON APCH TO OSU ARPT, POINTED OUT TO CMH TWR, THIS IS TO MAINTAIN SEPARATION. TWR TURNED SMT Y 300 DEG, ACFT CLIMBED TO 17' LEVELED. SEPARATION DETERIORATED TO 2 NM AT 17' 3 E OF OSU BEFORE SMT Y WAS FOUND STILL ON TWR FREQ. TO RESOLVE, TWR MUST FOLLOW SOP.
