Narrative:
Operational error report XXX overflt small aircraft X was not transponder equipped, and therefore was being manually tracked with ARTS IIIA. Radar antenna on airport, data tag dropped off as small aircraft X proceeded over airport and controller involved forgot to manually reacquire aircraft. Failure/inability of equipment to automatic-acquire was major contributor to this operational error.

Title: SMA X AND MIL ACFT Y HAD LESS THAN STANDARD SEPARATION IN ARSA. NMAC. SYSTEM ERROR.
Narrative: OPERATIONAL ERROR REPORT XXX OVERFLT SMA X WAS NOT XPONDER EQUIPPED, AND THEREFORE WAS BEING MANUALLY TRACKED WITH ARTS IIIA. RADAR ANTENNA ON ARPT, DATA TAG DROPPED OFF AS SMA X PROCEEDED OVER ARPT AND CTLR INVOLVED FORGOT TO MANUALLY REACQUIRE ACFT. FAILURE/INABILITY OF EQUIPMENT TO AUTO-ACQUIRE WAS MAJOR CONTRIBUTOR TO THIS OPERATIONAL ERROR.
