Narrative:

Cpr X level heading 270 degrees. Small aircraft Y departed F67 climbing to level 30. The airplanes got within 200 ft. Cpr X was given a climb clearance. Callback conversation with reporter revealed the following information: the reporter stated he had made a radar handoff to the departure controller with cpr Y, an light transport. The departure controller had not followed facility good operating practice when he released small aircraft Y, and small aircraft. Small aircraft Y should have been assigned vector heading 190 to maintain 2000 ft. The departure controller had forgot small aircraft Y conflict alert activated and cpr Y was issued climb to 4000 ft. Evasive action was not taken by the flcs. Operational error was filed.

Narrative: CPR X LEVEL HDG 270 DEGS. SMA Y DEPARTED F67 CLBING TO LEVEL 30. THE AIRPLANES GOT WITHIN 200 FT. CPR X WAS GIVEN A CLB CLRNC. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED HE HAD MADE A RADAR HDOF TO THE DEP CTLR WITH CPR Y, AN LTT. THE DEP CTLR HAD NOT FOLLOWED FACILITY GOOD OPERATING PRACTICE WHEN HE RELEASED SMA Y, AND SMA. SMA Y SHOULD HAVE BEEN ASSIGNED VECTOR HDG 190 TO MAINTAIN 2000 FT. THE DEP CTLR HAD FORGOT SMA Y CONFLICT ALERT ACTIVATED AND CPR Y WAS ISSUED CLB TO 4000 FT. EVASIVE ACTION WAS NOT TAKEN BY THE FLCS. OPERROR WAS FILED.
