Narrative:

Air carrier X was at FL190 en route from aus to bpt. He entered my departure sector from over tnv. I was stopping all my department at FL190. Air carrier Y, I had stopped at FL180. When he was clear, I climbed him to FL220 underneath additional traffic. Air carrier Z checked on and I climbed him to FL180. I then climbed air carrier Y to FL230. Air carrier Z called and asked if center had higher altitude, that he was reaching FL180. I gave him his air carrier X traffic at 12 O'clock, 3 mi, eastbound at FL190. He said, 'roger, we have traffic in sight.' I then went back to air carrier Y and reissued FL230. 'Roger, climbing to FL230. We've got the traffic, air carrier Z.' I lost sep at that time with air carrier Z and air carrier X. I feel if air carrier Z had realized our (center) requirement (5 mi, 1000/2000') he would have questioned the clearance. He should have also known we can't use a visibility climb in the positive control area. I realize I should have heard the mistake in call signs, but as well as he should have. Why are controllers being held responsible for a pilot's mistake??? Even had I caught the mistake immediately, loss of sep had already occurred!! Due to this present system, I am decertified and the pilot gets off free. Please, please if at all possible, do something to change this injustice!!

Title: PLT READ BACK CLIMB CLRNC FOR ANOTHER ACFT AND CTLR FAILED TO CATCH ERROR WHICH RESULTED IN LESS THAN STANDARD SEPARATION.

Narrative: ACR X WAS AT FL190 ENRTE FROM AUS TO BPT. HE ENTERED MY DEP SECTOR FROM OVER TNV. I WAS STOPPING ALL MY DEPT AT FL190. ACR Y, I HAD STOPPED AT FL180. WHEN HE WAS CLR, I CLBED HIM TO FL220 UNDERNEATH ADDITIONAL TFC. ACR Z CHKED ON AND I CLBED HIM TO FL180. I THEN CLBED ACR Y TO FL230. ACR Z CALLED AND ASKED IF CENTER HAD HIGHER ALT, THAT HE WAS REACHING FL180. I GAVE HIM HIS ACR X TFC AT 12 O'CLOCK, 3 MI, EBND AT FL190. HE SAID, 'ROGER, WE HAVE TFC IN SIGHT.' I THEN WENT BACK TO ACR Y AND REISSUED FL230. 'ROGER, CLBING TO FL230. WE'VE GOT THE TFC, ACR Z.' I LOST SEP AT THAT TIME WITH ACR Z AND ACR X. I FEEL IF ACR Z HAD REALIZED OUR (CENTER) REQUIREMENT (5 MI, 1000/2000') HE WOULD HAVE QUESTIONED THE CLRNC. HE SHOULD HAVE ALSO KNOWN WE CAN'T USE A VIS CLB IN THE PCA. I REALIZE I SHOULD HAVE HEARD THE MISTAKE IN CALL SIGNS, BUT AS WELL AS HE SHOULD HAVE. WHY ARE CTLRS BEING HELD RESPONSIBLE FOR A PLT'S MISTAKE??? EVEN HAD I CAUGHT THE MISTAKE IMMEDIATELY, LOSS OF SEP HAD ALREADY OCCURRED!! DUE TO THIS PRESENT SYS, I AM DECERTIFIED AND THE PLT GETS OFF FREE. PLEASE, PLEASE IF AT ALL POSSIBLE, DO SOMETHING TO CHANGE THIS INJUSTICE!!
