Narrative:
Small transport X inbound to land at bna. The track on the aircraft dropped off the target 10 mi east of bna. I forgot the aircraft and he called back 45 mi south of bna in ZME's airspace. ZME idented him and directed him to hunstville to land. No flight progress strip was passed to my radar position as a system of memory jogging to remind me of the aircraft. Situation could be prevented if flight progress strip was passed to remind controller of aircraft when computer system fails.

Title: APCH CTL ALLOWED SMT X TO ENTER CENTER AIRSPACE WITHOUT COORD. OPERATIONAL DEVIATION.
Narrative: SMT X INBND TO LAND AT BNA. THE TRACK ON THE ACFT DROPPED OFF THE TARGET 10 MI E OF BNA. I FORGOT THE ACFT AND HE CALLED BACK 45 MI S OF BNA IN ZME'S AIRSPACE.
