Narrative:
We were cleared to cross 40 east of muskegon at 23000' (clearance received about muskegon). The copilot was off the frequency and on #2 radio talking to his friends on unicom at traverse city and didn't tell me he was off the air. I copied the clearance on my pad but didn't put it in the altitude warning window. The second officer heard the clearance and didn't challenge me about the new altitude not being put in the window. The last leg of a 4 day trip and all 3 were fatigued. The teamwork I normally have and openness in the cockpit was not present partly because, the coplts responses throughout the month were conversation stoppers and not conducive to good communication. I felt a break down in the teamwork and didn't know how to reverse it. We had 3 different F/es for 4 trips and it contributed to the lack of teamwork. When we were 39 mi east, ATC asked us if we were going to make our crossing. Obviously no. Only 1 mi, they said do your best!

Title: ALT DEVIATION. CROSSING RESTRICTION NOT MET.
Narrative: WE WERE CLRED TO CROSS 40 E OF MUSKEGON AT 23000' (CLRNC RECEIVED ABOUT MUSKEGON). THE COPLT WAS OFF THE FREQ AND ON #2 RADIO TALKING TO HIS FRIENDS ON UNICOM AT TRAVERSE CITY AND DIDN'T TELL ME HE WAS OFF THE AIR. I FELT A BREAK DOWN IN THE TEAMWORK AND DIDN'T KNOW HOW TO REVERSE IT. WE HAD 3 DIFFERENT F/ES FOR 4 TRIPS AND IT CONTRIBUTED TO THE LACK OF TEAMWORK.
