Narrative:
First officer was flying the aircraft. We were given a crossing 30 mi southwest of ind (vhp) at 11000'/250 KTS. We were flying V12 eastbound to kelly intersection and then direct ind. The first officer had asked the captain if he could put vhp VOR on the captain's side since the first officer was navigating off shb VOR inbound to kelly intersection. The captain responded in the affirmative and selected the inbound radial to vhp VOR on his side. The first officer assumed that he had changed the VOR channel also, but he had not because he was under the impression the first officer was asking if the first officer could change the frequency and assumed the first officer made the change himself. The first officer was calculating the crossing using the captain's DME which was still on shb. Anyway, we missed the altitude restriction by approximately 12-15 mi or 5000'. We also missed the turn from kelly intersection direct ind by a few mi. The screw up resulted from poor communication from the first officer and lack of followup on navigation selection. No incident occurred and ATC did not comment nor did we!

Title: NAVIGATION DEVIATION AND ALT CROSSING RESTRICTION NOT MET.
Narrative: F/O WAS FLYING THE ACFT. WE WERE GIVEN A XING 30 MI SW OF IND (VHP) AT 11000'/250 KTS. WE WERE FLYING V12 EBND TO KELLY INTXN AND THEN DIRECT IND. THE F/O HAD ASKED THE CAPT IF HE COULD PUT VHP VOR ON THE CAPT'S SIDE SINCE THE F/O WAS NAVIGATING OFF SHB VOR INBND TO KELLY INTXN. THE CAPT RESPONDED IN THE AFFIRMATIVE AND SELECTED THE INBND RADIAL TO VHP VOR ON HIS SIDE. THE F/O ASSUMED THAT HE HAD CHANGED THE VOR CHANNEL ALSO, BUT HE HAD NOT BECAUSE HE WAS UNDER THE IMPRESSION THE F/O WAS ASKING IF THE F/O COULD CHANGE THE FREQ AND ASSUMED THE F/O MADE THE CHANGE HIMSELF. THE F/O WAS CALCULATING THE XING USING THE CAPT'S DME WHICH WAS STILL ON SHB. ANYWAY, WE MISSED THE ALT RESTRICTION BY APPROX 12-15 MI OR 5000'.
