Narrative:
While performing a service check on the aircraft. A possible hydraulic leak was noted on an access panel at bottom of right hand engine nacelle. Both engine cowlings were opened to find a possible source, however, none was found. Then the bottom access panel was removed and fluid was found on this panel. However, after thorough cleaning and inspection leak was considered to be residual. The access panel was reinstalled and cowlings closed. Service check was completed and signed off. Aircraft left the gate and took off. Later we found out that this aircraft was returning to dulles. Upon its arrival at the gate the right hand outboard engine cowling was found all bent up. Apparently the cowling was not secured properly and came open in flight. Supplemental information from acn 148678. Upon inspection it appeared that the loose cowling severed an oil pressure switch. The engine ported its oil supply through the broken switch.

Title: LTT COMMMUTER AIRLINER RETURNED TO IAD AND LANDED WHEN LOSS OF OIL AND PRESSURE REQUIRED ENGINE SHUT DOWN.
Narrative: WHILE PERFORMING A SVC CHK ON THE ACFT. A POSSIBLE HYD LEAK WAS NOTED ON AN ACCESS PANEL AT BOTTOM OF R HAND ENG NACELLE. BOTH ENG COWLINGS WERE OPENED TO FIND A POSSIBLE SOURCE, HOWEVER, NONE WAS FOUND. THEN THE BOTTOM ACCESS PANEL WAS REMOVED AND FLUID WAS FOUND ON THIS PANEL. HOWEVER, AFTER THOROUGH CLEANING AND INSPECTION LEAK WAS CONSIDERED TO BE RESIDUAL. THE ACCESS PANEL WAS REINSTALLED AND COWLINGS CLOSED. SVC CHK WAS COMPLETED AND SIGNED OFF. ACFT LEFT THE GATE AND TOOK OFF. LATER WE FOUND OUT THAT THIS ACFT WAS RETURNING TO DULLES. UPON ITS ARR AT THE GATE THE R HAND OUTBOARD ENG COWLING WAS FOUND ALL BENT UP. APPARENTLY THE COWLING WAS NOT SECURED PROPERLY AND CAME OPEN IN FLT. SUPPLEMENTAL INFO FROM ACN 148678. UPON INSPECTION IT APPEARED THAT THE LOOSE COWLING SEVERED AN OIL PRESSURE SWITCH. THE ENG PORTED ITS OIL SUPPLY THROUGH THE BROKEN SWITCH.
