Narrative:
I was assigned first officer pm. We were cleared to descend to fl 120 on the zamora 3C with an expected transition to the ILS 32L at lemd. 5 to 10NM beyond tobek at FL120 ATC queried us about why we had not held at tobek and gave us a vector to return to tobek and hold. Prior to reaching tobek we received vectors to the south and east with a descent to join the 32L ILS at lemd. The remainder of the approach; landing and taxi to the gate were uneventful.upon returning from break I verified the waypoints and altitudes on the FMC. I did not see a hold at tobek in the FMC and did not receive holding instructions at tobek from ATC. There was no discontinuity in the route. I failed to see the warning about the clearance limit at tobek on the approach plate.in the future I need to be more diligent about looking for warnings on approach plates. Perhaps the FMC data base could be altered to place a hold at tobek in order to emphasize the clearance limit.

Title: Air carrier flight crew reported missing a hold at a clearance limit on an international arrival because of ATC communication and charting issues.
Narrative: I was Assigned FO PM. I failed to see the warning about the clearance limit at TOBEK on the Approach Plate. In the future I need to be more diligent about looking for warnings on Approach Plates.
