Narrative:
While performing recurrent hazmat training; on the next to the last slide; it showed a hazmat card to be carried at all times. I was never informed or issued a hazmat training completion card. I notified [the trainer] of the oversight. I was issued a card via email later in the day. If I recall correctly the hazmat course and exam was issued as homework on the first or second weekend of training. In the initial course I do not recall the requirement to carry a hazmat card. After speaking with my classmates; all of us have the same issue.
