Narrative:
On a continuous duty overnight from ord to tvc to pln to tvc to ord scheduled or 3:30 block time and 11:54 duty time. Trip assigned to me at xa:00 local time for a show at xc:20 local I am on 24 hours a day reserve (so I don't know when to rest for trips like this). Blocked out of gate late at xd:35 local 1.6 hours later returned to gate for more fuel as we were getting low (at minimum fuel) still waiting for departure from ord. Taxi back out at xf:22 local and after another long taxi finally takeoff with a new route of flight to dbq, dll, grb adds about 45 min to normal flight time. First officer leg into pln initial WX as clear 1 mi fog. On approach I receive new WX as 1/4 mi fog. We saw the airport at about 3.5 mi out but about at 20' AGL we hit the fog that was 1/4 mi visibility or less. At this point I told the first officer 'go around' because he was unstable in the transition to visibility. We missed the approach and shot a southbound approach for landing. I don't remember if WX was 1/4 mi inside or outside the OM. We were both so exhausted we were not thinking clearly enough to be flying an airplane but we both also know to refuse to fly because of exhaustion would mean a lot of explanations to dispatch, chief pilot and other company officials with most likely disciplinary action and black mark by your name. At the end of trip blocked 8.3 KTS and 12.20 duty time with about 15 min map in pellston.

Narrative: ON A CONTINUOUS DUTY OVERNIGHT FROM ORD TO TVC TO PLN TO TVC TO ORD SCHEDULED OR 3:30 BLOCK TIME AND 11:54 DUTY TIME. TRIP ASSIGNED TO ME AT XA:00 LCL TIME FOR A SHOW AT XC:20 LCL I AM ON 24 HRS A DAY RESERVE (SO I DON'T KNOW WHEN TO REST FOR TRIPS LIKE THIS). BLOCKED OUT OF GATE LATE AT XD:35 LCL 1.6 HRS LATER RETURNED TO GATE FOR MORE FUEL AS WE WERE GETTING LOW (AT MINIMUM FUEL) STILL WAITING FOR DEP FROM ORD. ON APCH I RECEIVE NEW WX AS 1/4 MI FOG. WE SAW THE ARPT AT ABOUT 3.5 MI OUT BUT ABOUT AT 20' AGL WE HIT THE FOG THAT WAS 1/4 MI VIS OR LESS. AT THIS POINT I TOLD THE F/O 'GO AROUND' BECAUSE HE WAS UNSTABLE IN THE TRANSITION TO VIS. WE MISSED THE APCH AND SHOT A SBND APCH FOR LNDG. I DON'T REMEMBER IF WX WAS 1/4 MI INSIDE OR OUTSIDE THE OM. WE WERE BOTH SO EXHAUSTED WE WERE NOT THINKING CLRLY ENOUGH TO BE FLYING AN AIRPLANE BUT WE BOTH ALSO KNOW TO REFUSE TO FLY BECAUSE OF EXHAUSTION WOULD MEAN A LOT OF EXPLANATIONS TO DISPATCH, CHIEF PLT AND OTHER COMPANY OFFICIALS WITH MOST LIKELY DISCIPLINARY ACTION AND BLACK MARK BY YOUR NAME. AT THE END OF TRIP BLOCKED 8.3 KTS AND 12.20 DUTY TIME WITH ABOUT 15 MIN MAP IN PELLSTON.
