Narrative:
After pushing back from gate at jax we started all 3 engines and taxied straight to runway 35 for a takeoff. The after start check was completed and all door lights were verified and tested by the second officer and me in accordance with established procedure. Normal taxi check was also accomplished. After takeoff passing through 1000' MSL, a turn to a heading of 280 degrees was given. After flap retraction and rolling out on 280 degree heading, the second officer advised that the aircraft was not pressurizing. I gave the aircraft to the first officer and instructed him to level at 4000' MSL and advise jax departure of the problem. While the second officer and I were referring to the abnormal procedure and after verifying for myself that all doors were closed on the second officer's door annunciator panel, we both noticed the east&east door light illuminate at approximately the same time. This explained the problem of the aircraft not being able to pressurize. We were on ZJX by this time and returned for a normal landing at jax. Maintenance confirmed the door was open and that maintenance was performed the night before on the aircraft at jax. I must stress that the second officer performed a normal preflight and that he confirmed that the east&east door was closed. Door annunciator was confirmed out by all 3 crew members before takeoff.

Title: ACR LGT EQUIPMENT ACCESS DOOR DISCOVERED OPEN AFTER TKOF. ACFT WOULD NOT PRESSURIZE RETURN LAND.
Narrative: AFTER PUSHING BACK FROM GATE AT JAX WE STARTED ALL 3 ENGS AND TAXIED STRAIGHT TO RWY 35 FOR A TKOF. THE AFTER START CHK WAS COMPLETED AND ALL DOOR LIGHTS WERE VERIFIED AND TESTED BY THE S/O AND ME IN ACCORDANCE WITH ESTABLISHED PROC. NORMAL TAXI CHK WAS ALSO ACCOMPLISHED. AFTER TKOF PASSING THROUGH 1000' MSL, A TURN TO A HDG OF 280 DEGS WAS GIVEN. AFTER FLAP RETRACTION AND ROLLING OUT ON 280 DEG HDG, THE S/O ADVISED THAT THE ACFT WAS NOT PRESSURIZING. WHILE THE S/O AND I WERE REFERRING TO THE ABNORMAL PROC AND AFTER VERIFYING FOR MYSELF THAT ALL DOORS WERE CLOSED ON THE S/O'S DOOR ANNUNCIATOR PANEL, WE BOTH NOTICED THE E&E DOOR LIGHT ILLUMINATE AT APPROX THE SAME TIME. THIS EXPLAINED THE PROB OF THE ACFT NOT BEING ABLE TO PRESSURIZE. WE WERE ON ZJX BY THIS TIME AND RETURNED FOR A NORMAL LNDG AT JAX.
