Narrative:
During the departure from hnl, the keola one, the captain did not make the climb restriction of 2500' which is required at the koko head 240 right. He was at 1450' when the right xed. He then accelerated to 290 KTS below 3000' and held 290 KTS through 10000'.

Title: REPORTER OBSERVED CAPT MISSING A CROSSING ALT ON DEP HNL AND EXCEEDING SPEED RESTRICTION BELOW 10000'. QUESTIONING CAPT RESULTED IN A DEROGATORY REPLY. FO TOOK HIMSELF OFF THE FLT AT NEXT STOP OSTENSIBLY BECAUSE OF PERSONALITY CONFLICT THAT WOULD AFFECT SAFETY OF FLT.
Narrative: DURING THE DEP FROM HNL, THE KEOLA ONE, THE CAPT DID NOT MAKE THE CLB RESTRICTION OF 2500' WHICH IS REQUIRED AT THE KOKO HEAD 240 R. HE WAS AT 1450' WHEN THE R XED. HE THEN ACCELERATED TO 290 KTS BELOW 3000' AND HELD 290 KTS THROUGH 10000'.
