Narrative:
Confusion concerning oceanic clearance. Poor ATC facility at gant river required hold at Z1 but till clearance obtained (hold outside limits). Should have been turned over to ny sooner for clearance (oceanic). I had left cockpit for rest room. When back I assumed oceanic had been obtained. Most ATC facs will verify oceanic clearance before you are turned over to oceanic (ny in this case). Should be a procedure ATC uses to confirm oceanic, say 20 mins before entry point. Also needs to have 'B' pay scale/'a' scale settled. Have noted 'almost' intentional plans to get 'a' scale pilots into trouble if possible. Many 'B' (some) scale seem to have intentional bad attitudes. This 'a' scale-'B' scale does not provide the proper cockpit environment.

Title: ACR DEPARTED HOLDING FIX ENROUTE WITHOUT PROPER ATC CLRNC. PLT BLAMES ATC AND FO.
Narrative: CONFUSION CONCERNING OCEANIC CLRNC. POOR ATC FAC AT GANT RIVER REQUIRED HOLD AT Z1 BUT TILL CLRNC OBTAINED (HOLD OUTSIDE LIMITS). SHOULD HAVE BEEN TURNED OVER TO NY SOONER FOR CLRNC (OCEANIC). I HAD LEFT COCKPIT FOR REST ROOM. WHEN BACK I ASSUMED OCEANIC HAD BEEN OBTAINED. MOST ATC FACS WILL VERIFY OCEANIC CLRNC BEFORE YOU ARE TURNED OVER TO OCEANIC (NY IN THIS CASE). SHOULD BE A PROC ATC USES TO CONFIRM OCEANIC, SAY 20 MINS BEFORE ENTRY POINT.
