Narrative:
Flight plan was picked up in buf on leg prior to incident. Both commanding officer and first officer had flown same trip pairing previously in the month. Both assumed routing was as previously flown ie, bwi daily intersection (ott 184 degree right to daily) J61 hubbs J193 cvi cvi 211 degree J121 chs J79 vrb blufi 4 mia. Clearance was: balance #5 af 5M departure 119.45 squawk 1416 as filed routing was never verified or questioned. Prior to level off ATC gave us choice of 350 fl at reduced speed or 310 fl west/O cruise limitation. FL310 was chosen. After departing washington center airspace, jax center requested route verification. This was first indication of route discrepancy. Flight plan route was then noted for the first time by cockpit crew. Dispatcher and washington center were both contacted by land line upon arrival at mia as per washington's request. Discrepancy was acknowledged and regrets offered by captain. Following recommendations could prevent recurrence: routing should be verbally verified between captain and first officer. Over-water routing should be brought to pilots attention prior to departure. ATC should verify routing when aircraft first deviates from expected routing. All parties in system should be reminded of tendency to relax vigilance when WX and other conditions are especially favorable.
