Narrative:
Triple INS unreliability at time of transition to otw. Under radar contact requested rtb and fuel dump area. Approved. Dumped to below maximum landing weight. Replaced two INS'. Continued flight again when cleared by ground personnel and ATC. Second flight uneventful met by flight manager at arrival problem explained and released aircraft had had previous write up affecting INS.

CONTINUED FLT AGAIN WHEN CLRED BY GND PERSONNEL AND ATC. SEC FLT UNEVENTFUL MET BY FLT MGR AT ARR PROB EXPLAINED AND RELEASED ACFT HAD HAD PREVIOUS WRITE UP AFFECTING INS.
