Narrative:
We were cleared for the approach and intercepting the localizer B/C from the east. Later approach advised us that we were west of the localizer course. At that time we noticed that both localizer frequency were selected to standby mode. We executed a missed approach and requested vectors for a second approach, this wasn't a good day. We were operating on a reduced rest schedule due to WX the previous day and had been on duty since early am this day, flown 3 hours including 1 approach to min, 40 mins holding on ground and 20 min airborne holding and as we proceeded to our alternate airport we got in a hurry and missed a very basic and almost second nature item (selecting the localizer frequency). My future actions will include an additional checklist derived from 'localizer' localizer-up/OM-on/course-selected. This will be concluded after 'cleared for approach' from ATC.
Original NASA ASRS Text
Title: FLT CREW OF LTT MAP WHEN CTLR ADVISED THEY WERE OFF APCH COURSE.
Narrative: WE WERE CLRED FOR THE APCH AND INTERCEPTING THE LOC B/C FROM THE E. LATER APCH ADVISED US THAT WE WERE W OF THE LOC COURSE. AT THAT TIME WE NOTICED THAT BOTH LOC FREQ WERE SELECTED TO STANDBY MODE. WE EXECUTED A MISSED APCH AND REQUESTED VECTORS FOR A SEC APCH, THIS WASN'T A GOOD DAY. WE WERE OPERATING ON A REDUCED REST SCHEDULE DUE TO WX THE PREVIOUS DAY AND HAD BEEN ON DUTY SINCE EARLY AM THIS DAY, FLOWN 3 HRS INCLUDING 1 APCH TO MIN, 40 MINS HOLDING ON GND AND 20 MIN AIRBORNE HOLDING AND AS WE PROCEEDED TO OUR ALTERNATE ARPT WE GOT IN A HURRY AND MISSED A VERY BASIC AND ALMOST SEC NATURE ITEM (SELECTING THE LOC FREQ). MY FUTURE ACTIONS WILL INCLUDE AN ADDITIONAL CHKLIST DERIVED FROM 'LOC' LOC-UP/OM-ON/COURSE-SELECTED. THIS WILL BE CONCLUDED AFTER 'CLRED FOR APCH' FROM ATC.
