Narrative:
After departing pnc in IMC early morning darkness, encountered severe rime ice. Shortly thereafter, passenger door opened, which in turn lost approach plates. Due to accumulation of airframe ice, was unable to hold assigned altitude, and with the loss of intended destination approach plates, declared an emergency. Returned to pnc with an uneventful landing.

Title: SMT CPR PLT DECLARES EMERGENCY AND REVERSES COURSE BACK TO DEP ARPT AS ACFT IS UNABLE TO MAINTAIN ALT DUE TO RIME ICE ACCUMULATION. AS A SIDE ISSUE, THE DOOR POPPED OPEN AND HE THEN LOST HIS APCH PLATES.
Narrative: AFTER DEPARTING PNC IN IMC EARLY MORNING DARKNESS, ENCOUNTERED SEVERE RIME ICE. SHORTLY THEREAFTER, PAX DOOR OPENED, WHICH IN TURN LOST APCH PLATES. DUE TO ACCUMULATION OF AIRFRAME ICE, WAS UNABLE TO HOLD ASSIGNED ALT, AND WITH THE LOSS OF INTENDED DEST APCH PLATES, DECLARED AN EMER. RETURNED TO PNC WITH AN UNEVENTFUL LNDG.
