Narrative:
I was being released for a flight from bdl to cle, with a second leg on to ind. The company has just changed to a new system for the dispatch release, flight plan, WX, etc, called foms. I was given the paperwork at bdl. As is common, I was given my copy attached with the rest of my papers, along with a copy of only the release to be returned with my signature on it. I signed that copy and gave it to the agent and started to look at the information. The forecast for cle had some cloud cover reported, but not enough to require an alternate. We had left cle earlier that day and agreed with the forecast. Everything in the paperwork seemed to check out. The fuel figure on the release, flight plan, fuel sheet, and weight sheet coincided with the amount on board the aircraft. We proceeded on our way and had an uneventful flight to cle. At cle, I went inside to get an update on the WX to ind. A copy of the release and flight plan were attached to the WX. I was surprised to see an alternate airport listed for the bdl-cle leg, and two alternates listed for the leg to ind. As I thought I had missed an amended release, I called bdl operations to see of they had received one. A new operations agent was on duty and he said that there was only the original release there that the captain (me) had signed. He said that release included an alternate for that leg to cle. In check my copy of the release, I then discovered that the date on it was X-21-X, and not Y-22-X. The operations agent had apparently had me sign the release from that day, but had given me the release and flight plan from the previous day. The agent had also prepared the fuel sheet and weight sheet from the previous days data. You can be sure that I will always check the date in the future and not just sign the paperwork given to me. Although an alternate airport was not actually required by the WX, I believe that once the flight was dispatched with one, I was not really legal to not have the fuel on board for that alternate. I believe it was the change to the new foms system that probably caused the agent to pull up the wrong days data. However, as PIC, it was my responsibility to catch the error.

Title: CAPT SIGNED FLT RELEASE BUT LATER REALIZED THE RELEASE WAS WRONG DATE AND WAS DATA FOR PREVIOUS DAYS SAME FLT.
Narrative: I WAS BEING RELEASED FOR A FLT FROM BDL TO CLE, WITH A SEC LEG ON TO IND. THE COMPANY HAS JUST CHANGED TO A NEW SYS FOR THE DISPATCH RELEASE, FLT PLAN, WX, ETC, CALLED FOMS. I WAS GIVEN THE PAPERWORK AT BDL. AS IS COMMON, I WAS GIVEN MY COPY ATTACHED WITH THE REST OF MY PAPERS, ALONG WITH A COPY OF ONLY THE RELEASE TO BE RETURNED WITH MY SIGNATURE ON IT. I SIGNED THAT COPY AND GAVE IT TO THE AGENT AND STARTED TO LOOK AT THE INFO. THE FORECAST FOR CLE HAD SOME CLOUD COVER RPTED, BUT NOT ENOUGH TO REQUIRE AN ALTERNATE. WE HAD L CLE EARLIER THAT DAY AND AGREED WITH THE FORECAST. EVERYTHING IN THE PAPERWORK SEEMED TO CHK OUT. AS I THOUGHT I HAD MISSED AN AMENDED RELEASE, I CALLED BDL OPS TO SEE OF THEY HAD RECEIVED ONE. A NEW OPS AGENT WAS ON DUTY AND HE SAID THAT THERE WAS ONLY THE ORIGINAL RELEASE THERE THAT THE CAPT (ME) HAD SIGNED. HE SAID THAT RELEASE INCLUDED AN ALTERNATE FOR THAT LEG TO CLE. IN CHK MY COPY OF THE RELEASE, I THEN DISCOVERED THAT THE DATE ON IT WAS X-21-X, AND NOT Y-22-X. THE OPS AGENT HAD APPARENTLY HAD ME SIGN THE RELEASE FROM THAT DAY, BUT HAD GIVEN ME THE RELEASE AND FLT PLAN FROM THE PREVIOUS DAY. THE AGENT HAD ALSO PREPARED THE FUEL SHEET AND WEIGHT SHEET FROM THE PREVIOUS DAYS DATA. YOU CAN BE SURE THAT I WILL ALWAYS CHK THE DATE IN THE FUTURE AND NOT JUST SIGN THE PAPERWORK GIVEN TO ME. ALTHOUGH AN ALTERNATE ARPT WAS NOT ACTUALLY REQUIRED BY THE WX, I BELIEVE THAT ONCE THE FLT WAS DISPATCHED WITH ONE, I WAS NOT REALLY LEGAL TO NOT HAVE THE FUEL ON BOARD FOR THAT ALTERNATE. I BELIEVE IT WAS THE CHANGE TO THE NEW FOMS SYS THAT PROBABLY CAUSED THE AGENT TO PULL UP THE WRONG DAYS DATA. HOWEVER, AS PIC, IT WAS MY RESPONSIBILITY TO CATCH THE ERROR.
