Narrative:

After fxe departure climbing through 6000', mia ATC said transponder reporting 500' error and requested altitude mode be turned off. They asked if we would be returning to mia tonight. We said yes. They told us we would need a waiver to return and gave us the phone # to call for the waiver. Upon arrival at ft myers page field, we (my student, I supervised) called FSS and explained our situation. Requested they call mia and fmy and pbi--all arsa's--for waivers for our return to home base for repairs. Initially mia said no. 30 mins later, mia agreed and we were given a waiver #. I believe that because mia was such a problem, the FSS man forgot to get the others. We assumed we only needed 1 waiver # and proceeded to leave fmy. Fmy departure was very upset and informed us we did not have a waiver to depart their arsa. We explained we had requested one and thought we had obtained one. We asked fmy to contact pbi to assist us in obtaining one there if we did not have one. He refused to even check with pbi. Before we got to the pbi arsa, we asked mia to amend our flight plan to avoid the pbi arsa, which they did. Next time, which I hope there isn't, I will call each facility myself.

Title: INSTRUCTOR AND STUDENT ON INSTRUMENT TRAINING FLT EXPERIENCE TRANSPONDER FAILURE AND RUN AFOUL OF ATC ON RETURN FLT.

THEY TOLD US WE WOULD NEED A WAIVER TO RETURN AND GAVE US THE PHONE # TO CALL FOR THE WAIVER. UPON ARR AT FT MYERS PAGE FIELD, WE (MY STUDENT, I SUPERVISED) CALLED FSS AND EXPLAINED OUR SITUATION. REQUESTED THEY CALL MIA AND FMY AND PBI--ALL ARSA'S--FOR WAIVERS FOR OUR RETURN TO HOME BASE FOR REPAIRS. INITIALLY MIA SAID NO. 30 MINS LATER, MIA AGREED AND WE WERE GIVEN A WAIVER #. I BELIEVE THAT BECAUSE MIA WAS SUCH A PROB, THE FSS MAN FORGOT TO GET THE OTHERS. WE ASSUMED WE ONLY NEEDED 1 WAIVER # AND PROCEEDED TO LEAVE FMY. BEFORE WE GOT TO THE PBI ARSA, WE ASKED MIA TO AMEND OUR FLT PLAN TO AVOID THE PBI ARSA, WHICH THEY DID. NEXT TIME, WHICH I HOPE THERE ISN'T, I WILL CALL EACH FAC MYSELF.
