Narrative:

Air carrier X was positioned at the north end of taxiway a when clearance for takeoff on runway 17 was issued by cos tower. A reply of flight cleared for takeoff runway 17 was read back by the first officer. A back taxi of approximately 100 ft and a 180 degree turn around was executed. Prior to adding power, tower reported that a restriction to remain south of runway 12 had been issued with the takeoff clearance. The first officer reported to the tower that a restriction had not been received by the cockpit crew nor had it been read back to the tower. I think this situation could have been avoided by 1) the tower making it clear that this was to be an intersection takeoff, 2) make sure of a complete readback by the cockpit crew and the tower. Supplemental information from acn 180226: the captain executed a back taxi of approximately 100 ft and did a 180 degree turn. Prior to adding power, tower reported that a restriction to remain south of runway 12 had been issued with the takeoff clearance. The first officer reported to the tower that a restriction had not been received by the cockpit crew nor had it been read back to the tower. In addition, no information was received by the cockpit that this was to be an intersection takeoff from the north end of taxiway a. To prevent such an occurrence, it must be assumed that any information not read back was not received, thus must be challenged by the issuing party.

Narrative: ACR X WAS POSITIONED AT THE N END OF TAXIWAY A WHEN CLRNC FOR TKOF ON RWY 17 WAS ISSUED BY COS TWR. A REPLY OF FLT CLRED FOR TKOF RWY 17 WAS READ BACK BY THE FO. A BACK TAXI OF APPROX 100 FT AND A 180 DEG TURN AROUND WAS EXECUTED. PRIOR TO ADDING PWR, TWR RPTED THAT A RESTRICTION TO REMAIN S OF RWY 12 HAD BEEN ISSUED WITH THE TKOF CLRNC. THE FO RPTED TO THE TWR THAT A RESTRICTION HAD NOT BEEN RECEIVED BY THE COCKPIT CREW NOR HAD IT BEEN READ BACK TO THE TWR. I THINK THIS SITUATION COULD HAVE BEEN AVOIDED BY 1) THE TWR MAKING IT CLEAR THAT THIS WAS TO BE AN INTXN TKOF, 2) MAKE SURE OF A COMPLETE READBACK BY THE COCKPIT CREW AND THE TWR.
