Narrative:

Air carrier XXX cleared for takeoff on runway 16R and was instructed to make an early right turn traffic was an small aircraft off departure end straight out. XXX departed and did not start the right turn until I told him to start right turn. XXX passed 1/2 mi behind small aircraft. XXX then informed me that he was very close to the small aircraft and I informed him that this was the reason for the early right turn. Nothing else was said at the time. We received a call a couple of hours later from the pilot of XXX to find out what happened. We informed him that he had been cleared for takeoff and to make an early right turn and was informed of traffic off departure end of the runway. The pilot then informed the tower that he came within 300 ft of the small aircraft and that he was going to file a near midair collision report and hung up. We pulled the tapes and filled out all the paperwork.

Title: ACR X FLC NON ADHERENCE TO ATC INSTRUCTIONS FAILED TO MAKE R TURN AFTER DEP HAD CONFLICT WITH SMA Y. PLTDEV.

Narrative: ACR XXX CLRED FOR TKOF ON RWY 16R AND WAS INSTRUCTED TO MAKE AN EARLY R TURN TFC WAS AN SMA OFF DEP END STRAIGHT OUT. XXX DEPARTED AND DID NOT START THE R TURN UNTIL I TOLD HIM TO START R TURN. XXX PASSED 1/2 MI BEHIND SMA. XXX THEN INFORMED ME THAT HE WAS VERY CLOSE TO THE SMA AND I INFORMED HIM THAT THIS WAS THE REASON FOR THE EARLY R TURN. NOTHING ELSE WAS SAID AT THE TIME. WE RECEIVED A CALL A COUPLE OF HRS LATER FROM THE PLT OF XXX TO FIND OUT WHAT HAPPENED. WE INFORMED HIM THAT HE HAD BEEN CLRED FOR TKOF AND TO MAKE AN EARLY R TURN AND WAS INFORMED OF TFC OFF DEP END OF THE RWY. THE PLT THEN INFORMED THE TWR THAT HE CAME WITHIN 300 FT OF THE SMA AND THAT HE WAS GOING TO FILE A NMAC RPT AND HUNG UP. WE PULLED THE TAPES AND FILLED OUT ALL THE PAPERWORK.
