Narrative:
We were getting ready to depart when ground guide reported that we were cleared to start engines. The first officer told the ground controller that we were ready to start engines. The ground controller did not answer immediately, and when he did answer he admonished the first officer for calling only 2 mins after he had said we would be ready to start in 5 mins per the operating procedures. He then cleared us to start. After starting engines we received clearance to taxi, so I released the brakes. Then I heard the ground guide telling me to stop over the interphone, and I parked the brakes immediately. After properly releasing the ground guide and getting a proper salute, we proceeded to taxi. For some reason I thought that I had cleared the ground guide and gotten a salute, but I had not. Fortunately all I got was a dirty look from the guide man. I was once again reminded that it does not take much of a diversion of attention to cause a dangerous situation.

Title: PIC OF ACR LGT FORGETS TO OBTAIN ALL CLR SALUTE FROM CGP PRIOR TO BRAKE RELEASE.
Narrative: WE WERE GETTING READY TO DEPART WHEN GND GUIDE RPTED THAT WE WERE CLRED TO START ENGS. THE FO TOLD THE GND CTLR THAT WE WERE READY TO START ENGS. THE GND CTLR DID NOT ANSWER IMMEDIATELY, AND WHEN HE DID ANSWER HE ADMONISHED THE FO FOR CALLING ONLY 2 MINS AFTER HE HAD SAID WE WOULD BE READY TO START IN 5 MINS PER THE OPERATING PROCS. HE THEN CLRED US TO START. AFTER STARTING ENGS WE RECEIVED CLRNC TO TAXI, SO I RELEASED THE BRAKES. THEN I HEARD THE GND GUIDE TELLING ME TO STOP OVER THE INTERPHONE, AND I PARKED THE BRAKES IMMEDIATELY. AFTER PROPERLY RELEASING THE GND GUIDE AND GETTING A PROPER SALUTE, WE PROCEEDED TO TAXI. FOR SOME REASON I THOUGHT THAT I HAD CLRED THE GND GUIDE AND GOTTEN A SALUTE, BUT I HAD NOT. FORTUNATELY ALL I GOT WAS A DIRTY LOOK FROM THE GUIDE MAN. I WAS ONCE AGAIN REMINDED THAT IT DOES NOT TAKE MUCH OF A DIVERSION OF ATTN TO CAUSE A DANGEROUS SITUATION.
