Narrative:
Small transport X was advised by the center of a separation error. Small transport X was level at 230. Apparently air carrier Y was climbing to 330. We were on a collision course. Another controller saw the conflict and immediately turned air carrier Y. Czyz center has advised us that both aircraft were in their assigned areas. ATC made the error and a full investigation has begun. We were unaware of the near collision until notified by operations investigation. Both targets merged on the radar screen!

Title: SMT X HAD LTSS FROM ACR Y. SYS ERROR.
Narrative: SMT X WAS ADVISED BY THE CENTER OF A SEPARATION ERROR. SMT X WAS LEVEL AT 230. APPARENTLY ACR Y WAS CLBING TO 330. WE WERE ON A COLLISION COURSE. ANOTHER CTLR SAW THE CONFLICT AND IMMEDIATELY TURNED ACR Y. CZYZ CENTER HAS ADVISED US THAT BOTH ACFT WERE IN THEIR ASSIGNED AREAS. ATC MADE THE ERROR AND A FULL INVESTIGATION HAS BEGUN. WE WERE UNAWARE OF THE NEAR COLLISION UNTIL NOTIFIED BY OPS INVESTIGATION. BOTH TARGETS MERGED ON THE RADAR SCREEN!
