Narrative:
During preflight inspection the crew discovered the left main wheel break keys not aligned to the brake rotor slots. This wheel assembly was reinstalled and the aircraft released for service. The left main wheel was installed by myself. To the best of my knowledge the wheel was installed properly in accordance with manufacturer's instructions. The wheel assembly is built by XXX. I understand that this wheel assembly is susceptible to this problem.
