Narrative:
Captain unintentionally started takeoff roll without receiving flight attendant ready signal and with flight attendant still standing in aisle. Aircraft had back taxied on runway to takeoff position and flight attendant had been given 2 PA's to prepare for takeoff. Before takeoff checklist was completed noting cabin alert step not yet complete. After aircraft turned into takeoff position, first officer was queried whether flight attendant signal had been received and in some unknown manner captain received impression that it had been. Error came to captain's attention in conversation with flight attendant during flight. Reported to management by captain and allegedly by call to company from 1 or more passenger. Written and verbal reprimands. Occurrence related to other pilots.
