Narrative:
During a self audit of company training records it was discovered that I have exceeded the 12 month time period since my last recurrent ground training under far 135.343. Contributing factors: upon returning from a leave of absence, I assumed added responsibilities with the company. We experienced multiple personnel manning problems, and I was tasked to make a major report for the company. My mind was occupied with those problems. I forgot about recurrent ground training. My name does not appear on the training due and overdue listing. If it appeared on an earlier listing, I did not see it due to the foregoing. The company did not follow up to ensure that my training was scheduled and completed. They relied solely on the local instructor and employee to do the training. Action taken: I completed recurrent training on the same day that the problem was discovered. The company has recognized the lack of followup and is addressing the problem. Recommendation: company management should require training to be prescheduled on a specific date and require notification by telephone that it has been accomplished. If confirmation is not received by the due date, the company should follow up by telephone to take the pilot off line.

ACTION TAKEN: I COMPLETED RECURRENT TRAINING ON THE SAME DAY THAT THE PROBLEM WAS DISCOVERED. THE COMPANY HAS RECOGNIZED THE LACK OF FOLLOWUP AND IS ADDRESSING THE PROBLEM. RECOMMENDATION: COMPANY MGMNT SHOULD REQUIRE TRAINING TO BE PRESCHEDULED ON A SPECIFIC DATE AND REQUIRE NOTIFICATION BY TELEPHONE THAT IT HAS BEEN ACCOMPLISHED. IF CONFIRMATION IS NOT RECEIVED BY THE DUE DATE, THE COMPANY SHOULD FOLLOW UP BY TELEPHONE TO TAKE THE PLT OFF LINE.
