Narrative:
The flap takeoff aural warning sounded when the throttles were advanced for takeoff. We immediately discontinued the takeoff and discovered the flaps/slats were not set for takeoff. After clearing the runway, we completed the checklists and proceeded with a normal takeoff. Although I consider both of us to be conscientious of procedures and checklists, we made the mistake of not completing all checklists. However, I believe we were victims of the circumstances and a potentially flawed taxi checklist which has a break for a delayed engine start. While parked at the runup area, we received the takeoff numbers and started the other engine. We set the takeoff speeds, flap takeoff selector and maximum settings but overlooked the flap/slat lever. We then proceeded with the taxi checklist after the delayed engine start portion and unknowingly felt we had completed the whole checklist. I don't think either one of us will ever make this mistake again.
Original NASA ASRS Text
Title: FLAP WARNING WHEN PWR LEVERS ADVANCED CAUSED FLC TO DISCONTINUE TKOF, SET FLAPS, TKOF NORMAL.
Narrative: THE FLAP TKOF AURAL WARNING SOUNDED WHEN THE THROTTLES WERE ADVANCED FOR TKOF. WE IMMEDIATELY DISCONTINUED THE TKOF AND DISCOVERED THE FLAPS/SLATS WERE NOT SET FOR TKOF. AFTER CLRING THE RWY, WE COMPLETED THE CHKLISTS AND PROCEEDED WITH A NORMAL TKOF. ALTHOUGH I CONSIDER BOTH OF US TO BE CONSCIENTIOUS OF PROCS AND CHKLISTS, WE MADE THE MISTAKE OF NOT COMPLETING ALL CHKLISTS. HOWEVER, I BELIEVE WE WERE VICTIMS OF THE CIRCUMSTANCES AND A POTENTIALLY FLAWED TAXI CHKLIST WHICH HAS A BREAK FOR A DELAYED ENG START. WHILE PARKED AT THE RUNUP AREA, WE RECEIVED THE TKOF NUMBERS AND STARTED THE OTHER ENG. WE SET THE TKOF SPDS, FLAP TKOF SELECTOR AND MAX SETTINGS BUT OVERLOOKED THE FLAP/SLAT LEVER. WE THEN PROCEEDED WITH THE TAXI CHKLIST AFTER THE DELAYED ENG START PORTION AND UNKNOWINGLY FELT WE HAD COMPLETED THE WHOLE CHKLIST. I DON'T THINK EITHER ONE OF US WILL EVER MAKE THIS MISTAKE AGAIN.
