Narrative:
Failure to select takeoff flaps for takeoff. Problem arose during the use of the checklist in the final preparation for takeoff. Company procedures don't require active participation by both crew members for selection of flaps for takeoff. Contributing factors: 1) waiting for flight attendant to prepare cabin for takeoff (as required by checklist). 2) FAA en route checks being conducted by FAA added to the stress of the otherwise normal routine operation. 3) checklist not requiring action by both crew members for selection of flap (first officer action/response only). 4) discovered on takeoff by aircraft oral warning. 5) takeoff flaps then selected by first officer and were set by 70 KTS (normal V1 speed was over 100 KTS).

Title: LTT FLC BEGINS TKOF WITH NO FLAPS.
Narrative: FAILURE TO SELECT TKOF FLAPS FOR TKOF. PROBLEM AROSE DURING THE USE OF THE CHKLIST IN THE FINAL PREPARATION FOR TKOF. COMPANY PROCS DON'T REQUIRE ACTIVE PARTICIPATION BY BOTH CREW MEMBERS FOR SELECTION OF FLAPS FOR TKOF. CONTRIBUTING FACTORS: 1) WAITING FOR FLT ATTENDANT TO PREPARE CABIN FOR TKOF (AS REQUIRED BY CHKLIST). 2) FAA ENRTE CHKS BEING CONDUCTED BY FAA ADDED TO THE STRESS OF THE OTHERWISE NORMAL ROUTINE OP. 3) CHKLIST NOT REQUIRING ACTION BY BOTH CREW MEMBERS FOR SELECTION OF FLAP (FO ACTION/RESPONSE ONLY). 4) DISCOVERED ON TKOF BY ACFT ORAL WARNING. 5) TKOF FLAPS THEN SELECTED BY FO AND WERE SET BY 70 KTS (NORMAL V1 SPD WAS OVER 100 KTS).
