Narrative:
While flying karlo 4 arrival to phx from las (bld KARLO4, jan/92) intercepted 310 degree radial from phx instead of 310 radial from iwa. Abq center detected error and inquired about the phx versus iwa station/radial. Captain was flying aircraft but both pilots misread arrival and selected phx versus iwa. We asked abq center if others had problems with arrival radials, the answer was 'a whole bunch'. Upon further inspection, the problem arose because of 2 events: 1) the old karlo arrival was totally referenced from the srp VOR with no use of the iwa involved. 2) the depiction of the new karlo shows the 130 degree radial reference displayed 2 inches away form the iwa station reference (see highlighted areas on enclosed plate). The narrative of the bld KARLO4 is accurate but the depiction is easily confused. Especially since the old arrival was flown with exclusive reference to srp. Recommendation: when a new arrival changes the primary navigation reference, the 'changes' section at the bottom of the plate should so state (see highlighted area with new comments).

Title: ACR MLG TRACK HDG DEV ON STAR INTO PHX.
Narrative: WHILE FLYING KARLO 4 ARR TO PHX FROM LAS (BLD KARLO4, JAN/92) INTERCEPTED 310 DEG RADIAL FROM PHX INSTEAD OF 310 RADIAL FROM IWA. ABQ CTR DETECTED ERROR AND INQUIRED ABOUT THE PHX VERSUS IWA STATION/RADIAL. CAPT WAS FLYING ACFT BUT BOTH PLTS MISREAD ARR AND SELECTED PHX VERSUS IWA. UPON FURTHER INSPECTION, THE PROBLEM AROSE BECAUSE OF 2 EVENTS: 1) THE OLD KARLO ARR WAS TOTALLY REFED FROM THE SRP VOR WITH NO USE OF THE IWA INVOLVED. 2) THE DEPICTION OF THE NEW KARLO SHOWS THE 130 DEG RADIAL REF DISPLAYED 2 INCHES AWAY FORM THE IWA STATION REF (SEE HIGHLIGHTED AREAS ON ENCLOSED PLATE). THE NARRATIVE OF THE BLD KARLO4 IS ACCURATE BUT THE DEPICTION IS EASILY CONFUSED. ESPECIALLY SINCE THE OLD ARR WAS FLOWN WITH EXCLUSIVE REF TO SRP.
