Narrative:

Flying medium large transport aircraft. Changes in altimeter setting accomplished by push button which changes reference from standard (2992) to qnh. Cleared from FL180 to 12000 MSL. Leaving FL180 I pushed the altimeter button to reference altimeter to preset qnh value of 30.38. Shortly after level off at 12000 other pilot noted we were about 450 ft high (ie, 12450 ft vice 12000). I looked at my display and saw my altimeter was still set to standard (29.92). I pushed my altimeter button again, got the qnh value and immediately descended to the correct altitude (12000 MSL). Although I remember pushing the button out of FL180 I did not visually verify that the altimeter was in the qnh mode. Needless to say this will definitely be included in my scan from now on.

I LOOKED AT MY DISPLAY AND SAW MY ALTIMETER WAS STILL SET TO STD (29.92). I PUSHED MY ALTIMETER BUTTON AGAIN, GOT THE QNH VALUE AND IMMEDIATELY DSNDED TO THE CORRECT ALT (12000 MSL). ALTHOUGH I REMEMBER PUSHING THE BUTTON OUT OF FL180 I DID NOT VISUALLY VERIFY THAT THE ALTIMETER WAS IN THE QNH MODE. NEEDLESS TO SAY THIS WILL DEFINITELY BE INCLUDED IN MY SCAN FROM NOW ON.
