Narrative:

On an inbound flight a maintenance problem occurred. The problem was written up at dfw (destination). Maintenance appeared to fix the problem, signed the logbook, returned the aircraft to service. Aircraft was flown to act (waco) on a scheduled flight. During a post-flight walk around the maintenance problem recurred. We notified dispatch about the problem. The flight was cancelled. Soon after a 'maintenance' number was issued to the captain. He was called by the chief pilot and ordered to fly the aircraft back to dfw. Upon arrival at dallas we swapped aircraft. A couple of hours later after a round trip to wichita falls (sps) the captain was relieved of duty for breech of company policy regarding 'maintenance' numbers.

Title: ACR LTT ACFT OPERATED IN NON COMPLIANCE WITH THE ACFT MAINT PROCS.

Narrative: ON AN INBOUND FLT A MAINT PROBLEM OCCURRED. THE PROBLEM WAS WRITTEN UP AT DFW (DEST). MAINT APPEARED TO FIX THE PROBLEM, SIGNED THE LOGBOOK, RETURNED THE ACFT TO SVC. ACFT WAS FLOWN TO ACT (WACO) ON A SCHEDULED FLT. DURING A POST-FLT WALK AROUND THE MAINT PROBLEM RECURRED. WE NOTIFIED DISPATCH ABOUT THE PROBLEM. THE FLT WAS CANCELLED. SOON AFTER A 'MAINT' NUMBER WAS ISSUED TO THE CAPT. HE WAS CALLED BY THE CHIEF PLT AND ORDERED TO FLY THE ACFT BACK TO DFW. UPON ARR AT DALLAS WE SWAPPED ACFT. A COUPLE OF HRS LATER AFTER A ROUND TRIP TO WICHITA FALLS (SPS) THE CAPT WAS RELIEVED OF DUTY FOR BREECH OF COMPANY POLICY REGARDING 'MAINT' NUMBERS.
