Narrative:

I accepted a handoff on air carrier X going to lax. I made a handoff to lax TRACON on air carrier X. Air carrier X reported on frequency and I descended him and told him to contact lax approach as I thought a handoff had been accomplished. At the boundary I dropped the track on air carrier X. A few moments later lax TRACON called and said air carrier X was not handed off. The sector radar symbol (P for pomona sector) had just been changed to M. This was the first I had been exposed to it and I do not remember being briefed that this had changed. I might have been looking for the familiar 'P'; didn't see it and mistook the 'M' for lax TRACON's '2' symbol. This could be contributory.

Title: ACR X UNCOORD AIRSPACE ENTRY.

Narrative: I ACCEPTED A HDOF ON ACR X GOING TO LAX. I MADE A HDOF TO LAX TRACON ON ACR X. ACR X RPTED ON FREQ AND I DSNDED HIM AND TOLD HIM TO CONTACT LAX APCH AS I THOUGHT A HDOF HAD BEEN ACCOMPLISHED. AT THE BOUNDARY I DROPPED THE TRACK ON ACR X. A FEW MOMENTS LATER LAX TRACON CALLED AND SAID ACR X WAS NOT HANDED OFF. THE SECTOR RADAR SYMBOL (P FOR POMONA SECTOR) HAD JUST BEEN CHANGED TO M. THIS WAS THE FIRST I HAD BEEN EXPOSED TO IT AND I DO NOT REMEMBER BEING BRIEFED THAT THIS HAD CHANGED. I MIGHT HAVE BEEN LOOKING FOR THE FAMILIAR 'P'; DIDN'T SEE IT AND MISTOOK THE 'M' FOR LAX TRACON'S '2' SYMBOL. THIS COULD BE CONTRIBUTORY.
