Narrative:
On 8/92, small aircraft X was in the traffic pattern at crw. The flight was being conducted as a training flight. We were doing numerous touch-north-goes and were in left traffic for runway 23. After turning a left base and still having not received touch and go clearance, I tried calling crw tower. I did not receive a reply and continued onto final approach. After still not being able to reach tower I was looking for light gun signals and instructed my student to go around. The go around was accomplished and we reestablished left traffic for 23. Throughout the lesson, we were using headsets. After not being able to hear tower and trying to switch radios, I went from the headset to speaker. After I switched to the speaker, radio contact with tower was reestablished. I asked tower if we had been cleared for the touch and go and they stated that we were not. On downwind, tower asked me to contact them on the ground and we made the next landing a full stop. After speaking with crw supervisor, I explained to him that I was unable to hear them and that I had a radio failure. He stated that we had turned into air carrier Y and they wanted to pursue the incident. In my opinion, I felt that I had done everything 'by the book.' I felt that tower should have advised air carrier Y that they were unable to talk to me and that they could have given me light gun signals on approach. Supervisor also stated that tower advised us to extend downwind. It should have been obvious that since I had not replied I must not have heard them. He felt if any blame was to go to anyone it would be the tower for not advising air carrier Y of the potential problem and for not using light gun signals.

Title: SMA X LOST COM IN TFC PATTERN HAD POSSIBLE NMAC WITH ACR Y. SEE AND AVOID CONCEPT.
Narrative: ON 8/92, SMA X WAS IN THE TFC PATTERN AT CRW. THE FLT WAS BEING CONDUCTED AS A TRAINING FLT. WE WERE DOING NUMEROUS TOUCH-N-GOES AND WERE IN L TFC FOR RWY 23. AFTER TURNING A L BASE AND STILL HAVING NOT RECEIVED TOUCH AND GO CLRNC, I TRIED CALLING CRW TWR. I DID NOT RECEIVE A REPLY AND CONTINUED ONTO FINAL APCH. AFTER STILL NOT BEING ABLE TO REACH TWR I WAS LOOKING FOR LIGHT GUN SIGNALS AND INSTRUCTED MY STUDENT TO GAR. THE GAR WAS ACCOMPLISHED AND WE REESTABLISHED L TFC FOR 23. THROUGHOUT THE LESSON, WE WERE USING HEADSETS. AFTER NOT BEING ABLE TO HEAR TWR AND TRYING TO SWITCH RADIOS, I WENT FROM THE HEADSET TO SPEAKER. AFTER I SWITCHED TO THE SPEAKER, RADIO CONTACT WITH TWR WAS REESTABLISHED. I ASKED TWR IF WE HAD BEEN CLRED FOR THE TOUCH AND GO AND THEY STATED THAT WE WERE NOT. ON DOWNWIND, TWR ASKED ME TO CONTACT THEM ON THE GND AND WE MADE THE NEXT LNDG A FULL STOP. AFTER SPEAKING WITH CRW SUPVR, I EXPLAINED TO HIM THAT I WAS UNABLE TO HEAR THEM AND THAT I HAD A RADIO FAILURE. HE STATED THAT WE HAD TURNED INTO ACR Y AND THEY WANTED TO PURSUE THE INCIDENT. IN MY OPINION, I FELT THAT I HAD DONE EVERYTHING 'BY THE BOOK.' I FELT THAT TWR SHOULD HAVE ADVISED ACR Y THAT THEY WERE UNABLE TO TALK TO ME AND THAT THEY COULD HAVE GIVEN ME LIGHT GUN SIGNALS ON APCH. SUPVR ALSO STATED THAT TWR ADVISED US TO EXTEND DOWNWIND. IT SHOULD HAVE BEEN OBVIOUS THAT SINCE I HAD NOT REPLIED I MUST NOT HAVE HEARD THEM. HE FELT IF ANY BLAME WAS TO GO TO ANYONE IT WOULD BE THE TWR FOR NOT ADVISING ACR Y OF THE POTENTIAL PROBLEM AND FOR NOT USING LIGHT GUN SIGNALS.
