Narrative:
Passing 15000 MSL, I decided that the seat belt sign could be turned off and reached for the rotary switch. Much to my surprise, the seat belt rotary switch was in the off position. We had read the before start checklist and both acknowledged the seat belt sign was on. The widebody transport and large transport have a rotary switch for the no smoking and seat belt signs. A cursory glance is not sufficient to determine that the switch is in the proper position. Beware of these 'modern' rotary switches.
Original NASA ASRS Text
Title: FLC OF ACR LGT ACFT INADVERTENTLY FAILED TO TURN ON THE SEATBELT SIGN PRIOR TO TKOF.
Narrative: PASSING 15000 MSL, I DECIDED THAT THE SEAT BELT SIGN COULD BE TURNED OFF AND REACHED FOR THE ROTARY SWITCH. MUCH TO MY SURPRISE, THE SEAT BELT ROTARY SWITCH WAS IN THE OFF POS. WE HAD READ THE BEFORE START CHKLIST AND BOTH ACKNOWLEDGED THE SEAT BELT SIGN WAS ON. THE WDB AND LGT HAVE A ROTARY SWITCH FOR THE NO SMOKING AND SEAT BELT SIGNS. A CURSORY GLANCE IS NOT SUFFICIENT TO DETERMINE THAT THE SWITCH IS IN THE PROPER POS. BEWARE OF THESE 'MODERN' ROTARY SWITCHES.
