Narrative:
While on IFR flight plan, passed the OM, clearance from approach was not received to talk with the tower until 3 mi inside OM. By then, the student should have made a missed approach because tower not received (student had made 3 errors, was approaching too fast, which gave him little time, had not turned on OM audio, and had wrong frequency for tower 121.7 instead of 120.7). Final correction to tower made on 120.7. Student finally followed instructions to make missed approach. In order to prevent this occurrence in the future I will not allow student to commit an error (although sometimes the only way people learn is to allow him to make a mistake sometime, however how far the mistake should go is hard to decide). Regardless, I will not allow students to proceed into air traffic area without correcting the error myself.

Title: PVT PLT IN TRAINING FOR INST RATING MAKES LATE FREQ CHANGE ON IAP ILS APCH. INSTRUCTOR PLT MAKES STUDENT PLT MAKE A MISSED APCH.
Narrative: WHILE ON IFR FLT PLAN, PASSED THE OM, CLRNC FROM APCH WAS NOT RECEIVED TO TALK WITH THE TWR UNTIL 3 MI INSIDE OM. BY THEN, THE STUDENT SHOULD HAVE MADE A MISSED APCH BECAUSE TWR NOT RECEIVED (STUDENT HAD MADE 3 ERRORS, WAS APCHING TOO FAST, WHICH GAVE HIM LITTLE TIME, HAD NOT TURNED ON OM AUDIO, AND HAD WRONG FREQ FOR TWR 121.7 INSTEAD OF 120.7). FINAL CORRECTION TO TWR MADE ON 120.7. STUDENT FINALLY FOLLOWED INSTRUCTIONS TO MAKE MISSED APCH. IN ORDER TO PREVENT THIS OCCURRENCE IN THE FUTURE I WILL NOT ALLOW STUDENT TO COMMIT AN ERROR (ALTHOUGH SOMETIMES THE ONLY WAY PEOPLE LEARN IS TO ALLOW HIM TO MAKE A MISTAKE SOMETIME, HOWEVER HOW FAR THE MISTAKE SHOULD GO IS HARD TO DECIDE). REGARDLESS, I WILL NOT ALLOW STUDENTS TO PROCEED INTO ATA WITHOUT CORRECTING THE ERROR MYSELF.
