Narrative:
I was the last to fly the aircraft in 2 nights ago. Yesterday, the plane did not fly, and today I was the first to fly the aircraft. Both days the cockpit ditching hatch was checked on preflight and during our origination checklist by both me and the captain. Everything was done correctly by the flight crew's standpoint. As we rotated on takeoff (100 ft AGL) the cockpit hatch actually came off the aircraft. When it came off, it totally missed the plane and landed on the airport. We circled and landed without incident. We really couldn't have done anything more to prevent it. I think in the future our operations manual should have us check it more thoroughly and check it in more than just our preflight and originating checklists.

Title: FLC OF AN LTT ACR ACFT LOST THE COCKPIT ESCAPE HATCH DOOR DURING TKOF.
Narrative: I WAS THE LAST TO FLY THE ACFT IN 2 NIGHTS AGO. YESTERDAY, THE PLANE DID NOT FLY, AND TODAY I WAS THE FIRST TO FLY THE ACFT.
