Narrative:
Departing runway 28 at sfo, crew misread pre departure clearance and flew SID as filed on flight plan. Layout of pre departure clearance is such that misreading is easy. 'As filed' is in plain english while departure name is coded. Crews receive clearance 'as filed' almost every flight and have a tendency to see what they expect to see.

Narrative: DEPARTING RWY 28 AT SFO, CREW MISREAD PDC AND FLEW SID AS FILED ON FLT PLAN. LAYOUT OF PDC IS SUCH THAT MISREADING IS EASY. 'AS FILED' IS IN PLAIN ENGLISH WHILE DEP NAME IS CODED. CREWS RECEIVE CLRNC 'AS FILED' ALMOST EVERY FLT AND HAVE A TENDENCY TO SEE WHAT THEY EXPECT TO SEE.
