Narrative:
Aircraft was assigned a crossing altitude at fix. Captain flying missed altitude by 3000 ft or 3 mi on other side. The second problem was we set up approach to washington national for lda/DME 18, but aircraft was cleared roslyn lda 18. On altitude miss, I contribute it to a mental error. I should not just trust the captain but back him up and keep him advised. On wrong approach set- up, I listened to ATIS, heard roslyn lda 18. Found the first lda 18 chart (lda/DME 1) and set it up. I did not identify the localizer, we had flags. Captain tried to identify with no luck. Advised ATC, they gave us localizer frequency for roslyn lda. We idented and ran the approach. I think the 2 lda's to runway 18 which are approximately 30 degrees off set, should be renamed. One should be changed to lda 'a'. Therefore pilots are less apt to get the wrong approach chart out for lda 18.
