Narrative:
Small transport X departed runway 31L on course, heading 280 and transferred to my frequency 125.25. I attempted to obtain clearance for higher altitude with N90 but was unable. I cleared the aircraft out of class B airspace about 12 mi west jfk and advised that radar services were terminated, resume own navigation. Then N90 advised he should contact an N90 frequency. I advised the pilot. Very quickly he came back to me. Advised I could do no more and again advised resume own navigation, frequency change approved. I observed the aircraft continue on its heading, approaching ewr airport. I advised ewr tower by pointout. I then observed an aircraft on final descending out of 2000 on converging course with small transport X. Immediately after, targets merged, I observed ewr traffic Y (air carrier) climbing above 2000 (mode C). When the targets merged, the altitudes were the same.

Title: ATCT TRACON ATCT INTRAFAC COORD SMT X HAD LTSS FROM Y. SYS ERROR. NMAC.
Narrative: SMT X DEPARTED RWY 31L ON COURSE, HDG 280 AND TRANSFERRED TO MY FREQ 125.25. I ATTEMPTED TO OBTAIN CLRNC FOR HIGHER ALT WITH N90 BUT WAS UNABLE. I CLRED THE ACFT OUT OF CLASS B AIRSPACE ABOUT 12 MI W JFK AND ADVISED THAT RADAR SVCS WERE TERMINATED, RESUME OWN NAV. THEN N90 ADVISED HE SHOULD CONTACT AN N90 FREQ. I ADVISED THE PLT. VERY QUICKLY HE CAME BACK TO ME. ADVISED I COULD DO NO MORE AND AGAIN ADVISED RESUME OWN NAV, FREQ CHANGE APPROVED. I OBSERVED THE ACFT CONTINUE ON ITS HDG, APCHING EWR ARPT. I ADVISED EWR TWR BY POINTOUT. I THEN OBSERVED AN ACFT ON FINAL DSNDING OUT OF 2000 ON CONVERGING COURSE WITH SMT X. IMMEDIATELY AFTER, TARGETS MERGED, I OBSERVED EWR TFC Y (ACR) CLBING ABOVE 2000 (MODE C). WHEN THE TARGETS MERGED, THE ALTS WERE THE SAME.
