Narrative:

During logbook review prior to flight, I noticed a write-up on the copilot's VOR. I also noticed the item had been signed off by maintenance, but I did not read the signoff. I also failed to notice the placard in the logbook which showed the copilot's VOR placarded inoperative which is not legal. After reaching cruise altitude, I realized we departed in violation of MEL and notified dispatch of the inoperative VOR. The problem was caused because I did not look carefully enough at the logbook signoff and the placard sheet. A more careful review of the logbook will prevent a recurrence in the future.

Title: AN ACR LGT FLEW WITH ONLY 1 VOR RECEIVER.

Narrative: DURING LOGBOOK REVIEW PRIOR TO FLT, I NOTICED A WRITE-UP ON THE COPLT'S VOR. I ALSO NOTICED THE ITEM HAD BEEN SIGNED OFF BY MAINT, BUT I DID NOT READ THE SIGNOFF. I ALSO FAILED TO NOTICE THE PLACARD IN THE LOGBOOK WHICH SHOWED THE COPLT'S VOR PLACARDED INOP WHICH IS NOT LEGAL. AFTER REACHING CRUISE ALT, I REALIZED WE DEPARTED IN VIOLATION OF MEL AND NOTIFIED DISPATCH OF THE INOP VOR. THE PROB WAS CAUSED BECAUSE I DID NOT LOOK CAREFULLY ENOUGH AT THE LOGBOOK SIGNOFF AND THE PLACARD SHEET. A MORE CAREFUL REVIEW OF THE LOGBOOK WILL PREVENT A RECURRENCE IN THE FUTURE.
