Narrative:

Misunderstood a position and hold clearance for a takeoff clearance and took off. I feel fatigue was a contributing factor. It was nearly the end of my fourth consecutive day on duty which was to become a 14 hour 11 min duty day (the 3 previous days were 15 hours 24 mins, 12 hours 45 mins and 13 hours 18 mins on duty). I was also working with a new first officer in an airplane I was fairly new to, and flying in a new area and may have been distracted by cockpit duties as well as pressure to get back on schedule.

Narrative: MISUNDERSTOOD A POS AND HOLD CLRNC FOR A TKOF CLRNC AND TOOK OFF. I FEEL FATIGUE WAS A CONTRIBUTING FACTOR.
