Narrative:
After takeoff, after the 'gear up' call, the captain popped his seat belt release and said, 'I gotta go bad -- you've got it!' startled by this event, just after liftoff, I totally forgot the turn from 012 degrees to 030 degrees. Bay departure called me 'heavy, verify you are on a heading of 030 degrees.' I replied 'negative, but coming right at this time!' we were 3600 ft above the ground at 6 DME on the sfo 020 degree radial. Prevention: go to the bathroom before takeoff! Supplemental information from acn 266798: captain had an immediate physical need to use restroom. Suspected bad food from lunch. Captain was back by 5000 ft MSL and had to leave twice more by 3500 ft MSL (he had bad diarrhea).

Title: AN ACR WDB FAILED TO TURN WHEN THE SID REQUIRED IT.
Narrative: AFTER TKOF, AFTER THE 'GEAR UP' CALL, THE CAPT POPPED HIS SEAT BELT RELEASE AND SAID, 'I GOTTA GO BAD -- YOU'VE GOT IT!' STARTLED BY THIS EVENT, JUST AFTER LIFTOFF, I TOTALLY FORGOT THE TURN FROM 012 DEGS TO 030 DEGS. BAY DEP CALLED ME 'HVY, VERIFY YOU ARE ON A HDG OF 030 DEGS.' I REPLIED 'NEGATIVE, BUT COMING R AT THIS TIME!' WE WERE 3600 FT ABOVE THE GND AT 6 DME ON THE SFO 020 DEG RADIAL. PREVENTION: GO TO THE BATHROOM BEFORE TKOF! SUPPLEMENTAL INFO FROM ACN 266798: CAPT HAD AN IMMEDIATE PHYSICAL NEED TO USE RESTROOM. SUSPECTED BAD FOOD FROM LUNCH.
