Narrative:

Flight was on final approach to 18R at clt. At approximately 1700 ft AGL, the PF called `landing gear, down, landing checks.' at between 1000-1200 AGL, the nose wheel was considered not down. An immediate go around was initiated. ATC was advised of the go around and to please stand by. Upon reaching a safe altitude, the captain called for the appropriate abnormal checklist. After completing the abnormal checklist, an emergency was declared in preparation of a nose gear up landing. Company procedures and checklists were accomplished prior to initiating another approach. On short final, ATC confirmed the absence of nose wheel extension. Evacuate/evacuation would be imminent. Touchdown and rollout were normal until the nose made contact with the pavement. Maximum brake effort was used to shorten stop distance. Dense smoke filled the cockpit. By opening the right cockpit window, I was able to evacuate/evacuation a great deal of the smoke. The captain commanded the evacuate/evacuation at the complete stop. Appropriate checklists were accomplished. I checked the area outside the aircraft underneath the right cockpit window before egressing out the window to assist passenger evacuate/evacuation out the right forward service door. No injuries were reported among the 89 people aboard. Success of the described incident is primarily due to following prescribed company published procedure and checklists. Secondly, excellent use of cockpit resource management assisted in a maximum team effort.
