Narrative:

Was flying heading 352 degrees at 4500 ft en route to rio vista, ca. Saw cessna head on at approximately 1/4 mi. Immediate right rudder and full aileron to the right barely got me out of the way. TRACON notified me that aircraft was passing me. Closest near midair collision I have experienced. This is the second for me in this general vicinity. The last time, bay didn't see the twin that went over me within 100 ft. Maybe there is a problem with radar in this area. Other pilot said he was descending through 4800 ft when we passed. Other aircraft was from major flight school at sjc doing instrument work. Student was under hood at time. Supplemental information from acn 273029: I had him descend at 500 FPM rate of descent to 3000 ft for entry into sjc class D airspace. I put the landing light on for the descent and made the decision to contact bay approach. At around 5000 ft I saw a cessna 172 take a hard right evasive turn at about the same altitude. By the time I saw the cessna, it was too late to take an evasive maneuver.

Narrative: WAS FLYING HDG 352 DEGS AT 4500 FT ENRTE TO RIO VISTA, CA. SAW CESSNA HEAD ON AT APPROX 1/4 MI. IMMEDIATE R RUDDER AND FULL AILERON TO THE R BARELY GOT ME OUT OF THE WAY. THE LAST TIME, BAY DIDN'T SEE THE TWIN THAT WENT OVER ME WITHIN 100 FT. MAYBE THERE IS A PROB WITH RADAR IN THIS AREA. OTHER PLT SAID HE WAS DSNDING THROUGH 4800 FT WHEN WE PASSED. OTHER ACFT WAS FROM MAJOR FLT SCHOOL AT SJC DOING INST WORK. STUDENT WAS UNDER HOOD AT TIME. SUPPLEMENTAL INFO FROM ACN 273029: I HAD HIM DSND AT 500 FPM RATE OF DSCNT TO 3000 FT FOR ENTRY INTO SJC CLASS D AIRSPACE. I PUT THE LNDG LIGHT ON FOR THE DSCNT AND MADE THE DECISION TO CONTACT BAY APCH. AT AROUND 5000 FT I SAW A CESSNA 172 TAKE A HARD R EVASIVE TURN AT ABOUT THE SAME ALT. BY THE TIME I SAW THE CESSNA, IT WAS TOO LATE TO TAKE AN EVASIVE MANEUVER.
