Narrative:

I was acting as PF. Mke approach did not switch us over to mke tower. We did our approach down to minimums -- copilot called field in sight at decision ht. I looked up and called, 'flaps full, landing final items.' he called -- yaw damp away, flaps full, gear down and locked, cleared to land, then he said we are still on approach -- he switched to tower but he was unable to communication during the last 75 ft of approach and flare. This is technically lost communications. Tower said exit at B taxiway, contact ground 121.8. I acknowledged tower. Supplemental information from acn 274128: milwaukee cleared us to shoot the ILS to runway 1L about 4 mi outside of cappy (communication). The first officer, after receiving clearance, placed '900' in the altitude alert which signifies to the captain that we are cleared. Prevention: airline checklist could be completed sooner. ATC somehow forgot to hand crew off to local controller.
