Narrative:
Company failed to make appropriate flight plan. Had to return to departure airport, arrived at above maximum landing weight. Had to dump fuel. Failed to declare emergency. Suggest better training on fuel dumping. Fuel dumping is often not necessary. Dispatcher crew should have insisted on a holding rather than polluting the sea.

Narrative: COMPANY FAILED TO MAKE APPROPRIATE FLT PLAN. HAD TO RETURN TO DEP ARPT, ARRIVED AT ABOVE MAX LNDG WT. HAD TO DUMP FUEL. FAILED TO DECLARE EMER. SUGGEST BETTER TRAINING ON FUEL DUMPING. FUEL DUMPING IS OFTEN NOT NECESSARY. DISPATCHER CREW SHOULD HAVE INSISTED ON A HOLDING RATHER THAN POLLUTING THE SEA.
